Job summary

Eventus WholeHealth is seeking an Attending Physician to lead population health within our Outcomes-Centered Healthcare (OCH) model across client facilities. You will work closely with Advanced Practice Clinicians and facility leadership to deliver exceptional, team-based care and ensure accountability for patient outcomes.

The role may include Medical Directorship responsibilities, offering clinical leadership and oversight in one or more facilities.

Qualifications

• MD/DO degree with medical license in applicable state.
• Board certification in IM, FM or geriatrics preferred.
• CMD certification a plus; required within 2 years for Medical Directorship.
• Active, unrestricted medical license; DEA registration as required.
• 2–3 years clinical post-acute, long-term care, or value-based care experience.
• Experience collaborating with interdisciplinary teams and facility leadership.
• Familiarity with EMR systems; AthenaHealth preferred.

Responsibilities

• Lead population health within Eventus WholeHealth’s OCH model across client facilities.
• Collaborate with APCs and facility leadership to ensure high-quality, team-based care.
• Provide clinical leadership and potential Medical Directorship within facilities.
• Uphold professional standards and strengthen client relations.
