Job summary

Eventus WholeHealth seeks an Attending Physician to champion population health within the Outcomes-Centered Healthcare (OCH) model across client facilities. You will lead clinical teams, collaborate with APCs, and ensure high-quality, accountable, team-based care with a focus on professionalism in client relations.

In addition, there may be designated Medical Directorship duties providing clinical leadership and oversight in one or more facilities.

Qualifications

• Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) required.
• Board certification in Internal Medicine, Family Medicine, or Geriatrics preferred.
• CMD certification a plus; required within 2 years if serving in a Medical Directorship capacity
• Active and unrestricted medical license in applicable state(s); DEA registration as required
• Minimum 2–3 years of clinical experience in post-acute, long-term care, or value-based care settings preferred
• Experience collaborating with interdisciplinary teams and facility leadership
• Familiarity with EMR systems (AthenaHealth preferred) and care coordination workflow

Responsibilities

• Lead population health initiatives within Eventus WholeHealth's Outcomes-Centered Healthcare model across assigned facilities.
• Collaborate with Advanced Practice Clinicians and facility leadership to deliver high-quality, team-based care.
• Provide clinical oversight and, where applicable, Medical Directorship responsibilities.
• Foster professional relationships and ensure patient care excellence.
