Job summary

Eventus WholeHealth is seeking an Attending Physician to lead population health initiatives under the Outcomes-Centered Healthcare model. You will work closely with Advanced Practice Clinicians to deliver high-quality, team-based care across client facilities.

In addition, you may assume Medical Directorship responsibilities, offering clinical leadership and oversight in one or more sites. A strong focus on collaboration and professionalism is essential for success.

Qualifications

• MD or DO required; board certification in Internal Medicine, Family Medicine, or Geriatrics preferred.
• CMD certification a plus; required within 2 years if serving in Medical Directorship capacity.
• Active and unrestricted medical license; DEA registration as required.
• Minimum 2–3 years of clinical experience in post-acute, long-term care, or value-based care settings preferred.
• Experience collaborating with interdisciplinary teams and facility leadership.

Responsibilities

• Provide population-health stewardship within Eventus WholeHealth's Outcomes-Centered Healthcare model.
• Lead in client facilities and collaborate with APCs for high-quality, team-based care.
• May serve in Medical Directorship capacity with clinical leadership and oversight.
