Job summary

CenterWell Home Health is seeking a Speech-Language Pathologist to deliver in-home clinical services, evaluate patients, and develop plans of care across home and facility settings.

You will assess speech, language, voice, hearing, and swallowing disorders, administer diagnostic tests, and provide education to patients and families. A current SLP license and CPR certification are required, along with a valid driver’s license.

Qualifications

• Minimum six months experience as a speech-language pathologist
• Current and unrestricted license
• Current CPR certification
• A valid driver's license, auto insurance, and reliable transportation are required.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility
• Participate in the development and periodic review of the Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing to prevent, identify and minimize effects of such disorders
• Administer and interpret diagnostic tests and applications of therapeutic treatments including audiologic screening
• Observe, record and report changes in patient condition to supervisor/physician
• Provide instruction and training to patients in use of alternative communication systems when appropriate
• Provide counsel and instruction to patients, families and healthcare staff
• Maintain and submit documentation as required; prepare and submit written reports
• Participate in care coordination and discharge planning
• Maintain confidentiality of patient information
