Qualifications

• ASHA Certification in Speech-Language Pathology required.
• Minimum six months of experience as a speech therapist / SLP.
• Current and unrestricted license in Florida.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Evaluate, direct and provide speech/language pathology service to patients in the home or facility.
• Develop and review Plan of Treatment and Plan of Care.
• Assess and treat disorders of speech, voice, language, hearing and swallowing.
• Administer and interpret diagnostic tests and applications of therapeutic treatments including audiologic screening.
• Record changes and report to supervisor/physician.
• Provide instruction to patients in alternative communication systems when appropriate.
• Counsel patients, families and healthcare staff.
• Maintain and submit documentation and progress reports.
• Participate in care coordination and discharge planning.
• Maintain confidentiality of patient information.
