Job summary

The Institute for Family Health seeks a full-time Associate Program Director for the Connect for Health Program in Manhattan. You will provide clinical and case management services within an interdisciplinary team across multiple sites including church-based and LGBTQ youth programs.

You will supervise staff, coordinate with medical and behavioral health teams, oversee outreach and benefits enrollment, and help develop new services to meet community needs.

Qualifications

• Requires master’s level social work or equivalent and licensure in NYS.
• Two years of HIV/healthcare/social services experience required.
• Spanish or French/Creole language skills preferred.
• Strong supervisory and program management abilities are needed.

Responsibilities

• Provide crisis intervention, psychosocial assessments, suicide assessments and safety planning.
• Coordinate medical and psychiatric care, outreach, referrals, Medicaid enrollment and public benefits.
• Oversee medical case management and ensure documentation compliance.
• Supervise social workers, case managers and front desk staff; manage daily workflow.
• Develop and evaluate program goals and staff development initiatives.
