Job summary

Avance Care seeks an Associate Director of Population Health to design and drive value-based care strategies across its CIN network. The role emphasizes improving care quality, analytics, and operational effectiveness within multiple practices.

You will lead populations, develop dashboards, and coordinate with ACOs to optimize performance while managing reporting to commercial payers. A bachelor’s degree is required; advanced degrees are preferred.

Qualifications

• Minimum of 5+ years of population health management experience.
• Bachelor’s degree required; master’s or advanced degree preferred.

Responsibilities

• Manage value-based care, population health, and care management team for Avance Care and CIN participants.
• Deliver and execute strategic plan to monitor and improve value-based care measures across CIN practices.
• Collaborate with CIN Director and Account Managers to communicate with CIN participants.
• Develop and maintain a population health platform addressing clinical and financial improvements across care categories.
• Partner with ACOs to monitor provider and practice engagement and platform usage.
