Job summary

Services for the UnderServed is seeking an Assistant Program Director to supervise Case Management services ensuring quality care for adults experiencing homelessness and mental illness. In the absence of the Program Director, this role will supervise all program staff in conjunction with senior leadership.

The position requires a Bachelor's degree in Human Services or related field, with a minimum of two years of supervisory experience and direct work with homelessness and mental illness.

Qualifications

• Bachelor’s degree in Human Services or related field required.
• Minimum of two (2) years of supervisory experience.
• Experience with homelessness and mental illness.

Responsibilities

• Provide supervision of all program staff in relation to Case Management.
• Develop and implement individual client service plans with clinical staff and clients.
• Ensure contracts reflect goals of service plans and support intervention activities.
• Coordinate Annual Functional Assessment Review and submit recommendations.
• Maintain contact with other service agencies for client placements and program activities.
• Lead intake coordination for orientation and transitions of new residents.
• Participate in discharge planning and provide emergency coverage as needed.
• Represent the agency with community groups and assist with policy/ program design changes.
• Document supervisory meetings and maintain required records.
