Job summary

Fraser is Minnesota’s leader in autism, mental health, and disability services. Fraser seeks passionate individuals for leadership within direct care staff at group homes, offering a role that combines direct care with administrative responsibilities and staff leadership.

The Assistant Coordinator supports program operations and partners with Coordinators to ensure high-quality, person-centered services while promoting health, safety and independence.

Qualifications

  • High School diploma/equivalent preferred; post-secondary education preferred.
  • At least six months of experience with people who have developmental disabilities or related conditions preferred.
  • Valid Minnesota Driver’s License with acceptable record required.
  • Ability to pass MN DHS background study.
  • Commitment to promoting diversity, multiculturalism, and inclusion with focus on culturally responsive practice.
  • Physical ability to lift/carry up to 50 lbs. consistently.

Responsibilities

  • Provide direct support and person-centered care to individuals served.
  • Assist with medical, financial, and administrative management of assigned programs.
  • Coordinate appointments, transportation, schedules, and community activities.
  • Implement and monitor ISPs to support individual goals and needs.
  • Train, mentor, and support Direct Support Professionals.
  • Complete documentation and maintain compliance with agency policies and Minnesota 245D regulations.
  • Collaborate with families, guardians, healthcare providers, and interdisciplinary teams to coordinate services.
  • Identify opportunities to improve program quality and operational efficiency.

This position requires a self-motivated and responsible applicant with strong communication skills. The Assistant Coordinator supports program operations through a combination of direct care, administrative responsibilities, and staff leadership. This role partners with Coordinators to ensure high-quality, person-centered services while promoting the health, safety, and independence of individuals served.

Key Responsibilities:

  • Provide direct support and person-centered care to individuals served.
  • Assist with medical, financial, and administrative management of assigned programs.
  • Coordinate appointments, transportation, schedules, and community activities.
  • Implement and monitor Individual Service Plans (ISPs) to support individual goals and needs.
  • Train, mentor, and support Direct Support Professionals (DSPs).
  • Complete documentation and maintain compliance with agency policies and Minnesota 245D regulations.
  • Collaborate with families, guardians, healthcare providers, and interdisciplinary teams to coordinate services.
  • Identify opportunities to improve program quality and operational efficiency.

Pay, Locations, and Schedules:

  • $19/hour pay rate.
  • Weekend shift differential ($1.00 added to base pay rate for hours worked 10pm Friday through 9am Monday.)

Current openings include the following locations and schedules.

  • Apple Valley - Tuesday through Saturday 10am-6pm
  • Bloomington - Monday through Friday 9am-5pm
