Fraser is seeking a Program Coordinator to join its Community Living team in Minnetonka, MN. This leadership role provides guidance to staff, ensures high-quality services, and coordinates intakes and services with families and providers.
You will develop staff training, support professional growth, and maintain compliance with 245D regulations while overseeing one Supportive Living apartment setting.

Fraser is seeking a Program Coordinator to join our Community Living team! In this leadership role, you will support individuals served while providing guidance, supervision, and mentorship to staff to ensure high-quality services. You'll help create a positive team culture, support individualized treatment and service planning, and ensure compliance with 245D regulations.
Community Living includes both Supervised Living homes and Supportive Living apartments . This role will support one of Fraser's five Supportive Living apartment settings , where residents live more independently and do not require 24/7 supervision. The Program Coordinator will oversee one apartment complex.
