Job summary

Fraser is seeking an Assistant Program Manager to partner with the Program Manager in providing leadership, guidance, and operational support for our Supervised Living homes. You will coach and support staff, promote high-quality services, and help ensure individuals receive exceptional care in a safe and supportive environment.

You’ll play a key role in staff development, quality assurance, continuous improvement, and the implementation of best practices across the program.

Qualifications

• BA/BS in a field related to human services preferred.
• 245D qualification via HS Diploma + 4 years of direct care, or AA + 2 years, or BA + 1 year.
• One year of full-time work as a Designated Coordinator.
• Valid Minnesota Driver’s License with acceptable record.

Responsibilities

• Partner with Program Manager to provide leadership and oversight to Supervised Living homes.
• Cover temporary absences for the Program Manager with leadership support.
• Support orientation and ongoing training of Superviseds and staff.
• Mentor Coordinators through regular consults, shadowing, and paperwork reviews to ensure 245D compliance.
• Coordinate staffing with Staffing Coordinator based on individuals’ needs and budget.
• Promote collaboration within and between Supervised Living and other departments.
• Maintain relationships to increase engagement and coverage across sites.

Education

BA/BS in Human Services or related field

245D qualified (one of the listed options)

In this role, you'll coach and support staff, promote high-quality services, and help ensure individuals receive exceptional care in a safe and supportive environment. You'll play a key role in staff development, quality assurance, continuous improvement, and the implementation of best practices across the program.

Job duties:

• Partner effectively with Program Manager to provide leadership and oversight to the homes in the designated quad/region.
• Cover temporary absences for Program Manager with support from leadership.
• Support orientation and ongoing training of Supervised Living Coordinators, Assistant Coordinator; and DSPs as needed.
• Provide mentorship to Coordinators through regular consults, shadowing opportunities, team meeting support, and paperwork review to assure compliance with 245D requirements.
• Partner with Staffing Coordinator and colleagues to manage staffing, based on the needs of individuals served and budgetary constraints.
• Develop positive working relationships with staff and support teamwork within the quad/region to increase employee engagement, reduce turnover, and encourage coverage across sites.
• Recognize opportunities for and promote collaboration within and between Supervised Living and other departments.
• Coordinate services for one dedicated Supervised Living home in accordance with 245D licensing requirements and Community Residential Settings (CRS) standards.

Location, Schedule & Pay:

• Plymouth, Hopkins and Minnetonka areas
• Monday through Friday from 9 AM - 5 PM
• The starting pay range for this role is $53,000 - $58,000 annually depending on up qualifications.

Requirements:

• BA/BS in a field related to human services preferred.
• You must be 245D qualified in one of the following ways for this position:
  • HS Diploma or Equivalent AND four years of full-time work experience providing direct care services to persons with disabilities
  • Associate degree in Human Services or related field AND 2 years of full-time work experience (or equivalent) providing direct care services to persons with disabilities
  • BA/BS in Human Services or related field AND 1 year of full-time work experience (or equivalent) providing direct care services to persons with disabilities
• One year of full-time work experience as a Designated Coordinator required.
• Valid Minnesota Driver’s License with acceptable record required.
