Job summary

Breaking Ground in Queens seeks an Assistant Director, Programs, Case Management to guide street outreach, case management, and housing placement services. You will oversee a diverse team, coordinate with on-site psychiatric/street medicine staff, and ensure trauma‑informed care across all client interactions.

Reporting to the Director of Programs, you will lead program operations during regular business hours, with occasional after-hours coverage for emergencies and special projects.

Qualifications

• LMSW licensure in NY state is required.
• Minimum 2 years of supervisory experience.
• Experience with crisis intervention and risk assessments.
• Strong ability to collaborate with diverse stakeholders across agencies.
• Proficiency with Microsoft Word, Excel, Outlook, and Teams; willingness to learn new systems.
• A valid driver’s license (NY, NJ, CT, or PA) with good driving record preferred.
• Bilingual Spanish/English is preferred.

Responsibilities

• Supervise Clinical Coordinators, Housing Specialists, Housing and Reporting Specialists, and Street Medicine Coordinators.
• Provide regular, structured supervision sessions for staff.
• Support high-quality service delivery using Housing First and Harm Reduction models.
• Coordinate outreach, case management, and housing placement with partners and agencies.
• Ensure timely documentation of client contact and progress in the system.
• Lead development and oversight of individual service plans and crisis interventions.
• Liaise with external stakeholders to ensure responsive program operations.
