Spring Creek Village in Texas seeks an Assistant Director of Health and Wellness to lead the resident care team across assisted living and memory care. You will oversee health assessments, medication management, and day-to-day clinical operations, ensuring high-quality, compassionate care for residents.

This role requires LVN/LPN licensure, CPR, and prior leadership in senior care. You will coach staff, manage on-call coverage, collaborate with families and healthcare providers, and participate

Qualifications

• 1 year in senior care or related health programs.
• Management experience in Assisted Living Memory Care preferred.
• Current LVN/LPN license and CPR certification.
• Proficiency with EMR and MS Office.
• Strong leadership, communication, and customer service.
• Ability to work autonomously and manage priorities.

Responsibilities

• Lead and direct the resident care team in assisted living and memory care.
• Oversee resident health concerns, including assessments, medication management, and activities of daily living.
• Interview, train, and evaluate staff; handle on-call duties.
• Coordinate with the Health and Wellness Director to ensure service plans are followed and updated.
• Manage the health care of residents, perform assessments, and maintain contact with families and healthcare providers.
• Ensure proper medication management, documentation, and communication with families and medical professionals.
• Train and support memory care staff, ensuring a positive environment for residents and families.
• Be an active member of the community's emergency response team.

Location: Spring Creek Village, an Assisted Living with Memory Care senior community

Pay Rate: $71,310/year

Schedule: Sunday - Friday | 8:00 a.m. to 5:00 p.m.

(Manager on Duty weekend rotation, must be available holidays and weekends)

Primary Responsibilities

• Lead and direct the resident care team in assisted living and memory care.
• Oversee resident health concerns including assessments, medication management, and activities of daily living.

Minimum Qualifications

• 1 year in senior care or related health programs required. Management experience in Assisted Living Memory Care preferred.
• Graduate of an accredited LVN/LPN program.
• Current LVN/LPN license, CPR certification, and valid driver’s license.
• Proficiency with electronic medical records and Microsoft Office suite.
• Excellent leadership, communication, and customer service skills.
• Ability to work autonomously and manage priorities.
