Job summary

Village on the Park Onion Creek is seeking a compassionate Wellness Nurse to administer medications, oversee resident care, and support a team dedicated to dignity and person-centered service. The role requires an LVN/LPN license, BLS/CPR certification, and prior assisted living experience.

Weekend and holiday shifts are required, with opportunities for ongoing education. You’ll collaborate with physicians and families, perform ADL support, and ensure accurate clinical documentation.

Qualifications

• Current LVN/LPN license in the state of employment.
• BLS/CPR certification is required.
• 1 year of experience in Assisted Living or Long-Term Care recommended.
• Proficient in Electronic Health Records (EHR) and basic computer skills.

Responsibilities

• Administer medications safely and accurately per physician orders and regulatory standards.
• Oversee medication aides and conduct monthly medication reconciliation and audits.
• Coordinate daily staff assignments to ensure consistent resident care.
• Communicate effectively with physicians, families, and care teams regarding resident needs.
• Provide direct care including ADLs: bathing, dressing, grooming, toileting, mobility, and incontinence support.
• Respond to medical and behavioral emergencies with sound clinical judgment.
• Complete clinical documentation including assessments, evaluations, and service plans.
• Document incidents and communicate promptly with leadership.
• Monitor care team documentation for accuracy and completeness.
• Support orientation and ongoing training of care staff.
• Assist with scheduling in collaboration with leadership.
• Attend required in-services and continuing education.
• Maintain professional appearance and hygiene.

Qualifications:

• Current and valid LVN/LPN license in the state of employment
• Basic Life Support (BLS) / CPR certification
• Minimum 6 months to 1 year experience in Assisted Living or Long-Term Care
• Proficient in Electronic Health Records (EHR) and basic computer skills
• Strong verbal and written communication skills
• Demonstrated critical thinking and problem-solving abilities
• Knowledge of HIPAA regulations and compliance standards
• Ability to respond with patience and understanding to dementia-related behaviors
• Physical ability to perform essential job functions with or without reasonable accommodation
