Job summary

The Aspenwood Company in Austin, TX is seeking a Wellness Nurse (LVN/LPN) to administer medications, oversee resident care, and support a team focused on dignity, safety, and person-centered service. You will ensure accurate medication administration and collaborate with physicians, families, and care teams.

Weekend and holiday availability is required. Proficiency in EHR, HIPAA compliance, and dementia care is essential to maintaining high-quality care and communication across the organization.

Qualifications

• Current and valid LVN/LPN license in the state of employment.
• BLS/CPR certification required.
• 6 months to 1 year of experience in Assisted Living or Long-Term Care.
• Proficient in Electronic Health Records (EHR) and basic computer skills.
• Strong verbal and written communication skills.
• Knowledge of HIPAA regulations and compliance standards.
• Ability to respond with patience to dementia-related behaviors.
• Physical ability to perform essential job functions.

Responsibilities

• Administer medications safely and accurately per physician orders and regulatory standards.
• Oversee medication aides and conduct monthly medication reconciliation and audits.
• Coordinate daily staff assignments to ensure consistent resident care.
• Communicate effectively with physicians, families, and care teams regarding resident needs.
• Provide direct care including ADLs: bathing, dressing, grooming, toileting, mobility, and incontinence support.
• Respond to medical and behavioral emergencies with sound clinical judgment.
• Complete clinical documentation including assessments, evaluations, and service plans.
• Document incidents and communicate promptly with leadership.
• Monitor care team documentation for accuracy and completeness.
• Support orientation and ongoing training of care staff.
• Attend required in-services and continuing education.
• Maintain professional appearance and hygiene.
• Promote respectful and effective communication across teams and families.
• Perform other duties assigned to support resident care and operational excellence.
