Job summary

Lake County Health Department and Community Health Center is seeking an STI Assistant Program Coordinator to supervise program staff, manage day-to-day activities, and deliver direct patient services as needed.

In this role you will help plan, budget, monitor workloads, ensure documentation, and coordinate staff orientation and training, while developing partnerships with IDPH and community organizations.

Qualifications

• Master’s degree in a related field (Public Health, Health Education, Social Work, Psychology, Sociology, etc.).
• Bachelor’s degree in a related field and five years of relevant experience.
• Associate’s degree in a related field and seven years of relevant experience.
• Supervisory experience preferred.

Job description

The STI Assistant Program Coordinator is a valuable member of our Prevention team who provides day-to-day supervision and program management of assigned program staff and activities and provides direct patient services, as required.

Scheduled Hours: 40 hours per week

• Participates in program planning, budgeting, and review. Makes recommendations to improve program services and delivery.
• Monitors the supervisees’ workloads. In conjunction with other supervisory staff assures quality of and quantity of work is appropriate.
• Provides day-to-day problem solving and clinical supervision of staff. Acts as a resource person to staff.
• Conducts ongoing performance appraisals of supervisees, as well as monthly feedback to staff regarding their work via one on ones.
• Assures adequate documentation. Prepares beneficial and required narrative and statistical reports.
• Coordinates and/or participates in orientation of staff and interns.
• Identifies staff development needs and participates in the provision of such activities for supervisees.
• Develops/maintains positive working relationships with internal and external partners, IDPH and other community organizations including assisting with interpreting program needs and compliance.
• Displays a positive and respectful attitude, behavior, and demeanor toward management and co-workers in accomplishment of job duties.
• Provide direct services to clients in community, clinic and home settings according to need and established program protocols and procedures.
• Based on practice experience and analysis of service data, make recommendations for future program directions.
• Actively contributes to the planning and implementation of program evaluation activities.
• Complete and maintain all certifications required by the Illinois Department of Public Health STD Section to provide Comprehensive Risk Counseling services; STD Clinic services; Disease Investigation Services; and, appropriate surveillance functions.
• Provide comprehensive, competent and individualized one-on-one HIV/HCV/STD counseling and testing to individuals in an office and/or outreach environment as regulated through the Illinois Department of Public Health.
• Maintain daily reporting responsibilities for all job responsibilities and accurately maintain and update information in the STI Program’s database daily.
• Accurately maintain and update information in the Lake County Health Department & Community Health Center’s NextGen data system.
• Have oversight and management of a specific program area of responsibility including, but not limited to Syphilis surveillance, Hepatitis surveillance, STI services, Routine HIV Testing, HCV and PrEP case navigation, special events, community relations and outreach.
• Schedule appointments for the clinic services, counseling services and nursing services using approved protocols and algorithms.
• Complete and maintain all certifications required by the Illinois Department of Public Health HIV Section to provide Counseling, Testing and Referral services; Linkage to Care services, Group Prevention and Support services, HIV Navigation Services, Medication Adherence counseling, Surveillance Bases Services, Health Education and Risk Reduction Counseling services; and, provide HIV test results to individuals seen through the Program.
• Performs other duties as assigned.
