Job summary

Lake County Health Department and Community Health Center seeks an Advanced Practice Registered Nurse with Full Practice Authority to join the MAT Program. You will provide direct medical care, including admission H&Ps, prescribe medications, and develop treatment plans within licensure.

The role supports outpatient substance use treatment, collaborates with psychiatric providers, educates clients, and participates in crisis intervention and case management.

Qualifications

• Master's or Doctoral degree in Nursing required.
• FNP or ANP certification required per state law.
• Active Illinois APRN license with Full Practice Authority or endorsement.
• Active CSR and DEA registration required.
• Preferred: MAR experience in outpatient setting.

Job description

The Advance Practice Registered Nurse Full Practice Authority is a valuable member to our Medication Assisted Treatment Program (MAT) who provides direct medical clinical care of clients, including History and Physicals at admission and on an annual basis to include ordering labs, appropriate follow-ups, generating referrals, counseling clients, developing treatment plans and prescribing medications within the licensed scope of practice.

Our Medication Assisted Treatment Program (MAT) provides continued treatment for managing substance use disorders, specifically Opioid Use Disorders and Alcohol Use Disorders. All of our outpatient substance abuse programs, including our MAT Program, help clients learn to live without using drugs or alcohol. We pair clinical treatment with the support necessary to instill hope and empower our clients to lead meaningful and productive lives. Our MAT Program will utilize various medications in the treatment of the client’s substance use disorder, primarily Suboxone, Naltrexone (Vivitrol) and Sublocade, along with individual and group therapy, case management, and referrals to medical and psychiatric services.

Schedule: 20 hours a week

• Work hours will be determined based on agreed needs of the clinic

• Conducts admission and annual History and Physicals (H&Ps) as required by programmatic regulation. The primary responsibility is within the outpatient Substance Abuse Program/Medication Assisted Treatment programs.
• Routinely monitors, orders and prescribes necessary lab work and medication consistent with programmatic need, best practices, and scope of practice.
• Educates clients on diagnoses and recommend medications/treatments.
• Initiates and refills Medication Assisted Recovery (MAR) medications, including injections when applicable.
• Responds to inquiries, provides appropriate information, referrals and documentation, if necessary.
• Collaborate with psychiatric providers.
• Conducts crisis intervention/evaluation/referral to individuals who present themselves at the program.
• Communicates clearly, concisely and in a timely manner with treatment team and program staff.
• Collaborate with treatment team to review and update treatment plans.
• Facilitates with clients and collaborates with the treatment team to develop, review and update treatment goals and referrals both internally and externally.
• Provides education and direction to clients, family members and/or significant others.
• Train in and document clinical chart requirements to meet program and state standards.
• Maintains an awareness of and professional involvement/collaboration with community resources.
• Emergency coverage may be required for additional LCHD substance use disorder programs such as the Addiction Treatment Program Inpatient Withdrawal Management and Rehab units and the Women’s Residential Services Program.

Graduation prior to start date from a Master’s or Doctoral Degree in Nursing from a

• a. Family Nurse Practitioner (FNP) program accredited by either the CCNE or the ACEN, or
• b. Adult Nurse Practitioner (ANP) program accredited by either the CCNE or the ACEN

Preferred: At least 1 year of experience providing medication-assisted recovery (MAR) part-time, or equivalent

Certification and Licensure

Initial and continuing certification as a

• a. FNP by the ANCC, the American Academy of Nurse Practitioners National Certification Board (AANPCB) or other national certifying body as required by state law, or
• b. ANP by the ANCC, or other national certifying body as required by state law.

• Current active and unrestricted license from the State of Illinois as an Advanced Practice Registered Nurse with Full Practice Authority; or a letter from the Illinois Department of Professional Regulation stating that the application for licensure endorsement has been approved, with receipt of Illinois license following within six months of initial application date for endorsement.
• Current active and unrestricted Controlled Substance Registration (CSR) from the State of Illinois.
• Registration with the Drug Enforcement Agency (DEA) with current license in good standing.
