Job summary

Under the supervision of the Ryan White Case Management Coordinator, the Medical Case Manager coordinates the medical care of clients living with HIV/AIDS, in conjunction with the Medical Management Clinic at LCHD/CHC and works with clients to develop a supportive relationship while facilitating access and use of available services. Work involves direct client contact and is performed in accordance with state, local agency and grant policies and procedures.

Scheduled Hours: 37.5 hours per week

• Provides direct services to Ryan White clients in community, clinic and home settings according to need and established program protocols and procedures.
• Facilitates, organizes, and utilizes systems for tracking Ryan White client services and demographic information
• Completes and submits client intakes, eligibility assessments, ADAP applications, and HIV-related medical expenses in a timely manner into Provide database.
• Engages in care coordination, including providing necessary referrals, for optimal client outcomes and aids in the growth of client support groups.
• Conducts face-to-face assessment and reassessments to include history taking and an appraisal of the general overall appearance and demeanor of the client
• Development of an individualized service treatment plan
• Educates and counsels about HIV transmission, disease management, harm reduction, partner services, and mediation adherence to ART.
• Develops and maintains a basic knowledge of services available at the Lake County Health Department and Community Health Center (LCHD/CHC).
• Develops and maintains a basic awareness of the Social Determinants of Health and Trauma Informed Care to reduce health disparities within our communities.
• Participates in quality improvement activities to assess, monitor and improve program services.
• Maintain accurate and complete client files, treatment plans and program databases
• Participates in mandatory in-services and drills, attends staff meetings and other training, including appropriate age specific educational trainings offered on a regular basis for professional development.
• Reports all unusual occurrences that may involve the safety and wellbeing of the clients, clients and/or co-workers, directly to the supervisor and/or Safety Officer.
• Assists and encourages clients/clients in identifying self-management goals.
• Performs duties in a prioritized, organized and orderly manner to maximize clinic efficiency and productivity.
• Maintains orderliness and cleanliness of clinic and work area.
• Maintains accountability for work through timely submission of quantitative and qualitative data direct services.
• Actively contributes to the planning and implementation of program evaluation activities.
• Develops and maintains computer skills needed to complete assigned duties.
• Performs other duties as assigned.

• Master’s degree, bachelor’s degree, Registered Nurse, or associate’s degree.

  • With a Master’s/bachelor’s degree, a degree in a human services field (public health, health education, social work, psychology, sociology, early child development, etc.) or a degree in non-human services field and two years case management experience.

  • With an associate’s degree in a human services field, four years of work experience working with the public, preferably in a related field (public health, health education, social work, psychology, sociology, early child development, case management, etc.).

  • Bilingual Spanish written and verbal skills preferred, not required.
