Job summary

Central Clinic Behavioral Health is seeking an Assessor for the CDC Behavioral Health/ESP program in Hamilton, Ohio. This full-time role involves entering client data into the EHR, conducting diagnostic assessments, and developing treatment plans to address barriers to employment for individuals with mental health and substance use concerns.

The Assessor will collaborate with JFS and other providers, ensure timely documentation, and participate in ongoing training to strengthen service delivery.

Qualifications

• Master's degree in Social Work, Counseling or related field required.
• Ohio licensure in social work or counseling required.
• Substance use disorder (SUD) experience preferred.

Responsibilities

• Enter clients into the EHR and maintain data spreadsheets; administer drug screens as needed.
• Provide diagnostic assessments to address barriers to employment for clients with mental health and/or co-occurring disorders.
• Develop a treatment plan to address identified barriers and connect clients with the right providers.
• Staff cases with JFS and other providers; meet service delivery targets.
• Provide direct case management services including assessment, planning, daily living support, crisis intervention, and advocacy.

Education
