Job summary

Central Clinic Behavioral Health in Cincinnati, OH seeks a care coordinator to engage clients across children, youth and adults, coordinate services, and support multidisciplinary care planning.

Responsibilities include documenting services per Medicaid standards, coordinating referrals, maintaining knowledge of community resources, and advocating for client access to appropriate services. A Bachelor’s degree in social work, counseling, or related field is required; Ohio license preferred.

Qualifications

  • Bachelor's degree in social work, counseing, or related behavioral health field.
  • Experience working with children with behavioral health needs.
  • Valid driver's license, auto insurance and reliable transportation (if applicable)
  • Ohio license in either social work or counseling preferred.

  • Engage children/youth/adults in care.
  • Provide case coordination and care management for assigned clients.
  • Document services accurately in accordance with Medicaid and agency standards.
  • Participate in care planning and multidisciplinary meetings.
  • Collaborate with identified sources and service providers to complete timely referrals.
  • Maintain current knowledge of community resources that address the needs of individuals served.
  • Monitor client progress and reassess level of care needs.
  • Identify and resolve barriers to treatment engagement.
  • Maintain regular communications with stakeholders to coordinate services.
  • Advocate for client access to appropriate services and supports.
  • Serve as a connector to services based upon identified diagnostic assessments.
  • Support continuity of care across systems (behavioral health, school, child welfare, medical).
  • Perform other duties as assigned.
