Job summary

Lifepoint Health is seeking a Licensed Case Manager for the Acute Rehabilitation Unit at Kent Hospital in Warwick, RI. The role focuses on coordinating patient care, discharge planning, and collaboration with a multidisciplinary team to support high-quality outcomes.

Qualified candidates will hold RN or Social Work licensure (or rehab credentials) with CM certification preferred; two years of inpatient case management experience is highly valued.

Qualifications

• Current RN or Social Work licensure (LMSW, LCSW, LICSW) or Healthcare professional licensure (PT, SLP, OT) preferred.
• Certification in Case Management or Rehabilitation Nursing preferred (CCM, ARN, ACM, or RN-BC).
• Minimum of two years social work or case management experience in an inpatient setting highly preferred.

Responsibilities

• Coordinate and manage patient care to ensure high-quality outcomes and regulatory compliance.
• Provide assessment, care planning, coordination and ongoing evaluation of patient needs.
• Facilitate discharge planning and communication with patients, families and the interdisciplinary team.
