Qualifications

• Clinical degree and current license to practice as an OT, PT, RN, or SLP.
• Case management or rehabilitation nursing certification preferred (CCM, ARN, ACM, or ANCC RN-BC).
• Minimum of two years of social work or case management experience in an inpatient setting.
• English communication skills; additional languages helpful.

Responsibilities

• Coordinate patient care in an inpatient rehabilitation unit to ensure optimal outcomes.
• Conduct comprehensive assessments and develop individualized care plans.
• Collaborate with an interdisciplinary team and support discharge planning.
• Maintain accurate documentation and ensure regulatory compliance.
