Qualifications

• Bachelor’s degree required.
• Current RN or Social Work licensure or healthcare professional licensure (e.g., Respiratory Therapist, Physical Therapist, Speech-Language Pathologist, or Occupational Therapist).
• Case Management or Rehabilitation Nursing certification preferred (e.g., CCM, ARN, ACM, or RN-BC).
• Minimum of two years social work or case management experience in an inpatient setting; acute/rehabilitation hospital experience preferred.

Responsibilities

• Provide ongoing assessment, care coordination, plan implementation and evaluation of patient needs.
• Serve as patient advocate through resource utilization and discharge planning.
• Coordinate care options and communicate with patients and families in interdisciplinary teams.
• Complete IPoC documentation per CMS guidelines and workflow timelines.
• Coordinate weekly patient care team conferences to monitor and refine treatment plans.
• Collaborate with nursing, therapy and ancillary departments to ensure proper utilization and continuity of care.
