Job summary

Lifepoint Rehabilitation is seeking a Licensed Case Manager to coordinate care in Charlton Memorial Hospital's inpatient rehabilitation unit. The role focuses on assessments, care planning, and discharge coordination to ensure quality, cost-effective patient care.

Strong communication and licensure in OT/PT/RN/SLP are required. The position offers day shifts with a competitive hourly rate and comprehensive benefits.

Qualifications

• Effective oral and written communication skills in English.
• Certification in Case Management or Rehabilitation Nursing preferred (CCM/ARN/ACM or RN-BC).
• Minimum of two years of social work or case management in an inpatient setting preferred.

Responsibilities

• Coordinate patient care in an inpatient rehabilitation unit to optimize outcomes.
• Develop and implement individualized care plans and discharge plans.
• Collaborate with interdisciplinary teams and advocate for patients to ensure appropriate services and cost-effective, high-quality care.
