Job summary

6 Degrees Search is seeking an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based health organization in Washington state. This permanent, full-time role operates within a Patient Centered Medical Home model and collaborates closely with physicians, ARNPs, PAs, and nursing staff to deliver high-quality, comprehensive care across a diverse patient population.

Qualifications

• Graduate of an accredited ARNP program (required).
• Current RN and ARNP licensure in the State of Washington (required).
• Prescriptive authority from Washington State (required).
• Experience practicing in a broad primary care clinic setting (required).
• Experience in minor surgical techniques (preferred).
• Strong knowledge of the nursing process including assessment, planning, implementation, and evaluation.

Responsibilities

• Perform advanced clinical assessments and manage illnesses within ARNP scope.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management.
• Develop and implement individualized teaching plans to support patient health.
• Use independent clinical judgment based on evidence-based data.
• Maintain timely chart documentation (within 48 hours, 95% of the time).
• Meet productivity expectations (minimum 3,400 encounters per year, full-time).
• Provide care coordination through daily huddles in the PCMH model.
• Identify at-risk patient populations and monitor panel health.
• Empower patients through self-management education and discharge planning.
