SourcePro Search, LLC is seeking an experienced Advanced Registered Nurse Practitioner (ARNP) to join a patient-centered, community-based health organization in Washington. The role operates within the Patient Centered Medical Home model and collaborates with physicians, ARNPs, PAs, and nursing staff to deliver high-quality care to a diverse population.

The ARNP will perform advanced assessments, manage illnesses within scope, and develop individualized teaching plans to help patients achieve

Qualifications

• Graduate of an accredited ARNP program and WA licensure required.
• Prescriptive authority in Washington state required.
• Experience in a broad primary care setting preferred.

Responsabilités

• Perform advanced clinical assessments and manage conditions within ARNP scope.
• Collaborate with physicians, PAs, ARNPs and nursing staff on care management.
• Educate patients and discharge planning to support self-management.
• Maintain accurate chart documentation within 48 hours, 95% of the time.
• Meet productivity target of 3,400 encounters per year (FT).
