Job summary

Ridgemont Resources, Inc. seeks an Advanced Registered Nurse Practitioner to deliver patient-centered care in a community-based setting within the PCMH model. Washington licensure and prescriptive authority are required, with relocation assistance available.

You will collaborate with physicians, PAs, ARNPs, and nursing staff, perform advanced assessments, coordinate care, educate patients, and participate in quality improvement initiatives to enhance population health.

Qualifications

• Graduate of an accredited ARNP program.
• Current RN and ARNP licensure in Washington State.
• Prescriptive authority in Washington.
• Experience in broad primary care clinic setting.
• Experience in minor surgical techniques (preferred).
• Strong knowledge of the nursing process including assessment, planning, implementation, and evaluation.

Responsibilities

• Perform advanced clinical assessments and manage illnesses within ARNP scope of practice.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management.
• Develop and implement individualized teaching plans to support patients in achieving and maintaining optimal health.
• Use sound, independent clinical judgment in patient care decisions based on evidence-based data.
• Maintain timely and accurate chart documentation (within 48-hour timeline, 95% of the time).
• Complete fee slips with appropriately matching diagnosis and procedure codes.
• Meet productivity expectations (minimum 3,400 encounters per year, full-time equivalent).
• Provide care coordination through daily huddles as part of the PCMH model.
• Identify at-risk patient populations and monitor panel-specific population health.
• Empower patients through self-management education and discharge planning.
• Advocate for patients by explaining all procedures and treatment plans prior to execution.
• Participate in quality management, peer review, and departmental practice development.
• Support clinical education of other health professionals including student nurse practitioners.
• Maintain all required annual training (HIPAA, Cultural Diversity, BBP, Sexual Harassment).
