Food Management Search, Inc. is seeking an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based clinic in Washington state.

This full-time role operates within the Patient Centered Medical Home model, collaborating with physicians, ARNPs, PAs, and nursing staff to deliver high-quality, comprehensive care across a diverse patient population. You will perform advanced clinical assessments, manage illnesses within ARNP scope, and support patient education

Formación

• Graduate of an accredited ARNP program is required.
• Current RN and ARNP licensure in the State of Washington is required.
• Prescriptive authority from Washington State is required.
• Experience in a broad primary care clinic setting is required.

Responsabilidades

• Perform advanced clinical assessments and manage illnesses within ARNP scope.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management.
• Develop and implement individualized teaching plans for patients’ health.
• Maintain timely chart documentation (within 48 hours, 95% of the time).
• Meet productivity expectations (minimum 3,400 encounters per year).
• Provide care coordination through daily huddles in PCMH model.
• Identify at-risk populations and monitor panel health.
• Empower patients through self-management education and discharge planning.
