Top Echelon Software, LLC is seeking an experienced Advanced Registered Nurse Practitioner (ARNP) to join a patient-centered, community-based healthcare organization in Washington. The role works within the Patient Centered Medical Home model, collaborating with physicians, PAs, ARNPs, and nursing staff to deliver high-quality care across diverse populations.

Qualifications

• Graduate of an accredited ARNP program (required).
• Current RN and ARNP licensure in Washington (required).
• Prescriptive authority from Washington State (required).
• Experience practicing in a broad primary care clinic setting (required).

Responsibilities

• Perform advanced clinical assessments and manage illnesses within ARNP scope.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management.
• Develop personalized teaching plans to support patients in achieving optimal health.
• Use independent clinical judgment grounded in evidence-based data.
• Maintain chart documentation within 48 hours, 95% of the time.
• Complete fee slips with appropriate diagnosis and procedure codes.
• Meet productivity expectations (min 3,400 encounters per year) for full-time.
• Provide care coordination through daily huddles in the PCMH model.
• Identify at-risk populations and monitor panel health.
