Top Echelon Software LLC seeks an experienced Advanced Registered Nurse Practitioner to join a patient-centered, community-based health team in Washington. The role operates in a PCMH model and collaborates with physicians, ARNPs and nursing staff to deliver comprehensive care.

Qualifikationen

• Graduate of an accredited ARNP program.
• Current RN and ARNP licensure in the State of Washington.
• Prescriptive authority from Washington State.
• Experience practicing in a broad primary care clinic setting.
• Experience in minor surgical techniques is preferred.

Aufgaben

• Perform advanced clinical assessments and manage illnesses within ARNP scope.
• Collaborate with physicians, PAs, ARNPs, and nursing staff on patient care management.
• Develop and implement individualized teaching plans for patient health.
• Use independent clinical judgement based on evidence-based data.
• Maintain timely chart documentation (within 48 hours, 95% of the time).
• Complete fee slips with correct diagnosis and procedure codes.
• Meet productivity expectations (min. 3,400 encounters/yr full-time).
• Provide care coordination through daily huddles in PCMH model.
• Identify at-risk populations and monitor panel health.
• Empower patients through self-management education and discharge planning.
• Explain procedures and plans to patients before execution.
• Participate in quality management, peer review, and practice development.
• Support clinical education of students and staff.
• Maintain all required annual training (HIPAA, Cultural Diversity, BBP, Sexual Harassment).
