Solari is hiring for a 211 Program Specialist to respond to community calls, identify needs, and connect callers with resources via Virtual Case Management. You will document needs, referrals, and outcomes, ensuring timely connections and confidentiality.

The role emphasizes knowledge of health and social services in Arizona, strong communication, and the ability to work both in an office and remote setting as part of a hybrid schedule.

Qualifications

• GED or high school diploma required.
• 0–2 years in health and human services, public health, or healthcare.
• Call center experience highly desirable.
• Proficient with Microsoft Office and EHR systems.

Responsibilities

• Provide phone-based information, referral, and advocacy for health and social services.
• Document needs, referrals, and outcomes; use web-based systems for interactions.
• Identify gaps and contribute to 2-1-1 data reporting.
• Maintain confidentiality and professionalism with clients.
• Understand social services/resources in Arizona; follow standard practices.
• Perform duties as assigned.
