Job summary

Georgia Department of Human Services is seeking an APS Central Intake Specialist (Intake Case Manager 2) to support the Division of Aging Services. The role collects referrals, assesses eligibility, and documents in Harmony, with travel across Georgia as needed.

Hours are 10 am to 7 pm, with telework possibilities depending on position. The position requires a Bachelor’s degree and may require one year of related experience.

Qualifications

• Bachelor’s degree from an accredited college or university.
• One (1) year of experience at lower level APS Intake CM 1 (SSP180) or position equivalent.

Responsibilities

• Gathers referral information from persons making the maltreatment report via telephone interviews, written reports or face to face contacts.
• Assess APS referral information and determine if the situation meets APS criteria.
• Provide Information and Referral Services on cases not meeting APS criteria.
• Document referral information in Harmony thoroughly and clearly.
• For reports meeting criteria, document as an Investigation.
• For reports not meeting criteria, document as an Intervention.
• Clear the name of the person referred in Harmony to determine active history.
• Note findings in Harmony.
• Completes an acknowledgement letter to the reporter unless waived.
• Save the letter in the Central Intake Folder.

Education

Bachelor’s degree

One year of experience at APS Intake CM 1 or position equivalent
