Job summary

South Carolina Department of Social Services is seeking an APS Case Manager to perform professional case management for vulnerable adults, assess risk, coordinate services, and collaborate with community partners to ensure client safety.

You will manage caseloads, develop service plans with clients, coordinate with law enforcement and courts, and document thoroughly while supporting emergency response and protective actions across Richland County.

Qualifications

• Bachelor's degree in Social Work, Behavioral Science or Social Science.
• Bachelor's Degree in any other field with one year related experience.
• Master's Degree in Social Work, Behavioral Science or Social Services or higher can be substituted for required work experience.

Responsibilities

• Provide professional case management for vulnerable adults per DSS policy and APS ComPASS model.
• Assess risk, implement protective interventions, coordinate services while respecting client self-determination.
• Develop and coordinate service plans with clients to promote safety and stability.

Education

Bachelor's Degree in Social Work, Behavioral Science or Social Science

Bachelor's Degree in any other field with 1 year related experience

Master's Degree in Social Work, Behavioral Science or Social Services or higher
