Job summary

The South Carolina Department of Social Services is seeking an Adult Protective Services Case Manager to provide professional case management for vulnerable adults, assess risk, and coordinate protective interventions in accordance with DSS policy.

You will manage a caseload, collaborate with community partners and legal systems, and support emergency response efforts to maintain client safety and independence.

Responsibilities

• Performs professional case management services for vulnerable adults per DSS policy and APS ComPASS model.
• Assesses risk, implements protective interventions, and coordinates services for client safety.
• Manages caseloads, collaborates with community partners and legal systems, supports emergency response.
• Develops service plans with clients to promote safety and stability; arranges placements and supports.
• Coordinates with law enforcement and court system for protective actions and housing arrangements.
• Communicates with clients, families, providers, and staff to ensure effective service delivery.
• Assists in determining capacity for accessing long-term supports and financial resources.

Education

Bachelor’s degree (Social Work/Behavioral Science/Social Science)

Master’s degree (preferred)
