Job summary

The South Carolina Department of Social Services seeks an APS Case Manager in Columbia, SC. You will perform professional case management for vulnerable adults, assess risk, and coordinate protective services.

You will work with community partners and legal systems to ensure client safety and self-determination. Responsibilities include client home visits, service planning, court coordination when appropriate, and maintaining accurate documentation in electronic systems.

Qualifications

• Bachelor’s Degree in Social Work, Behavioral Science or Social Science.
• Master’s Degree in Social Work or related field can substitute for required experience.
• Valid driver’s license and access to a vehicle for field work.

Responsibilities

• Provide professional case management for vulnerable adults per DSS policy.
• Assess risk and implement protective interventions.
• Coordinate services to ensure client safety and self-determination.
• Maintain caseload, collaborate with partners and legal systems.
• Support emergency response efforts to maintain continuity of care.
• Provide service planning with clients for safety and stability.
