Job summary

The South Carolina Department of Social Services is seeking an APS Case Manager to protect and support vulnerable adults and families across the state. You will manage an assigned caseload, conduct risk assessments, coordinate services with partners, and document impact on client safety.

This role requires strong communication and collaboration with law enforcement, courts, and community providers. Responsibilities include home visits, service planning, and facilitating protective actions in

Qualifications

• Bachelor's in Social Work or related field; Master's preferred.
• Valid driver's license; field work readiness required.
• Experience with risk assessment and protective interventions.

Responsibilities

• Provide professional case management for vulnerable adults per policy and APS model.
• Assess risk, implement protective actions, coordinate services and document promptly.
• Manage caseload, collaborate with partners and support emergency response.
• Develop and coordinate service plans, including housing and supports.
• Communicate with clients, families, providers, and staff to ensure care.
• Coordinate with medical/legal professionals on capacity for supports.
• Perform other duties as required.
