Bluegrass Care Navigator is seeking a licensed APRN to provide direct participant care in a PACE Center setting. The role involves evaluating outcomes, coordinating with medical leadership, and guiding the plan of care for elderly participants within regulatory standards.

The APRN will work closely with the Chief Medical Officer and Medical Director, perform consultations, and ensure evidence-based practice while maintaining high-quality documentation and on-call coverage as needed.

Qualificações

• Must have graduated from an accredited master's degree nursing program.
• APRN certification as a Family Nurse Practitioner (FNP) or Adult Nurse Practitioner (ANP) and current state licensure.
• Minimum of three (3) years of general nursing experience; geriatrics preferred.
• Minimum of one (1) year of experience with frail/elderly populations.
• Strong clinical nursing skills, documentation, and CPR certification current.

Responsabilidades

• Provide direct participant care and evaluate outcomes.
• Establish and evaluate practice standards; provide nursing consultation to staff.
• Collaborate with the Medical Director and Chief Medical Officer to implement the participant-centered plan of care.
• See scheduled participants, walk-ins, and add-ons; provide telephone consultations as needed.
• Participate in quality improvement activities and manage patient care within scope of training.
