Job summary

BoldAge PACE is seeking a physician, NP, or PA to deliver primary care and continuous healthcare services to PACE participants. You will assess, plan, and provide care in alignment with PACE standards and work closely with the clinical care team to develop care plans and ensure continuity.

Role requires NJ licensure, CDS/DEA, and strong geriatric care skills. On-call coverage, medication management, and coordination with specialists are essential to success in this position.

Qualifications

• MD, DO, NP, APRN or PA eligible for NJ licensure.
• State CDS Certification and DEA Registration required.
• Board certification in Geriatrics preferred but not required.

Responsibilities

• Deliver high-quality primary care to PACE participants with comprehensive history and physicals.
• Collaborate with IDT and Medical Director to coordinate referrals and care plans.
• Manage participant medications and reconcile interactions with Pharmacist support.
• Provide 24/7 on-call coverage and ensure safe transitions during hospitalizations.
• Contribute to discharge planning and ongoing quality improvement initiatives.
