Job summary

Florida PACE Centers, Inc. is seeking an ARNP or PA to provide direct primary medical care and counseling for participants enrolled in the program. You will perform comprehensive histories and physicals for referrals and ongoing care, manage acute and chronic illnesses, and coordinate with the Medical Director and specialists.

The role requires licensure as an ARNP or PA, nursing background, and experience with frail elderly populations, with documentation and teamwork being key priorities.

Qualifications

• Graduate from an accredited school of nursing.
• Advanced training and certification as an ARNP or licensure as a PA.
• 2–3 years’ experience preferred; minimum of 1 year with frail elderly.
• Current ARNP/PA license by the Florida State Board of Nursing.

Responsibilities

• Provide direct primary medical care and counseling to participants enrolled in FLORIDA PACE CENTERS.
• Perform comprehensive histories and physical exams for new referrals and regular intervals.
• Manage acute, episodic, and chronic illnesses; coordinate with medical director and specialists.
• Maintain documentation in patient charts and participate in team meetings and committees.
