Job summary

NJ Palliative Care Solutions seeks Advanced Practice Nurses to deliver high-quality palliative care in inpatient and outpatient settings across central New Jersey, partnering with RWJBarnabas Health. The role emphasizes autonomy within a mission-driven model and close collaboration with clinical teams.

Ideal candidates have NJ APN licensure with prescriptive authority and at least 3 years in palliative care or related fields, with a flexible schedule and strong communication skills.

Qualifications

• Active New Jersey APN license with prescriptive authority
• Valid CDS and DEA registrations
• Minimum of 3 years’ clinical experience in palliative care, geriatrics, family medicine, or oncology preferred
• Strong clinical judgment, communication skills, and commitment to whole-person care
• Reliable transportation and a valid driver’s license are required

Responsibilities

• Deliver palliative care services across multiple settings including patients’ homes, SNFs, ALFs, and telehealth platforms
• Participate in both inpatient and outpatient care
• Conduct comprehensive assessments, manage complex symptoms, and lead advanced care planning discussions
• Collaborate with RWJBarnabas Health physicians, care managers, and community providers
• Flexible schedule supported by leadership and administrative infrastructure
