Job summary

RWJBarnabas Health is seeking an Inpatient Palliative Care Nurse Practitioner or Physician Assistant to join Jersey City Medical Center in Jersey City, NJ. The role focuses on high‑touch palliative care with structured mentorship and an interdisciplinary team, Monday–Friday with flexible swaps.

Requirements include active NJ NP or PA license, DEA+CDS, at least 2 years in palliative care, and a master’s degree.

Qualifications

• Must hold an active New Jersey NP or PA license with DEA & CDS registration or ability to obtain by start date.
• 2+ years of palliative care experience required.
• Palliative care certification (ACHPN/CHPN) preferred.

Responsibilities

• Deliver high‑quality, patient‑centered palliative care across hospital settings including ED and ICU.
• Lead goals‑of‑care and advance care planning discussions with patients and families.
• Coordinate with interdisciplinary teams and hospital services to integrate palliative medicine into care plans.
• Complete timely documentation aligned with clinical, billing, and quality standards.
• Focus on discharge planning and transitions to the next setting based on patient preferences.
