Job summary

Residential Transitions, Inc. (RTI) is hiring an ApartmentConnect Specialist to support tenants with mental health concerns and/or substance use disorders at Minnesota apartment complexes.

You will provide on-site, in-person support, coordinate with county services, and assist with paperwork and referrals to appropriate programs. Responsibilities include establishing trust with tenants, following up on referrals, and coordinating with a network of partners to promote housing stability and

Qualifications

• Degree in health- or human services-related field or equivalent.
• At least one year of experience with the target population (mental illness/substance use).
• Certification as Mental Health Certified Peer Specialist.
• Meet Minnesota DHS unlicensed personnel requirements (statutes 144A.43 to 144A.483).

Responsibilities

• Establish trusting relationships with tenants to facilitate services.
• Explore creative housing solutions to reduce homelessness risk.
• Follow up on referrals from Property Manager to assess eligibility for programs.
• Learn crisis and mental health services offered by DCSS and CRU.
• Coordinate with Embedded Social Worker and local partners to provide site-based support.
• Build partnerships with stakeholders to bring services on-site.
• Coordinate with community organizations for on-site events and resources.
