Job summary

Residential Transitions, Inc. (RTI) is hiring a Part-Time IHS Community Mental Health Worker in the Minneapolis–St. Paul metro area.

You will provide individualized home support with training, focused on teaching independent living and community skills, enabling clients to live fulfilling lives. The role reports to the Program Manager and involves caseload-based scheduling, potential travel within the metro area, and a meaningful, person-centered approach to care.

Qualifications

• Experience in mental health care settings (SMI/TBI) or related education.
• Ability to pass DHS and background checks.
• Own vehicle with valid license and willingness to travel within the metro area.

Responsibilities

• Provide 1:1 home support to clients.
• Teach independent living and community skills.
• Model transportation, finances, self-management, and daily living tasks.
• Facilitate socialization and community participation.
• Support healthy living habits and skill development.
• Collaborate with the Program Manager and team.
