Job summary

Catholic Charities of the Archdiocese of New York is hiring an Apartment Program Case Manager in the Bronx. You will support residents in learning life skills and help them live more independently through individualized service plans.

Responsibilities include coordinating medical, psychiatric, and entitlement services, visiting clients regularly, and maintaining timely progress notes. Requires a degree with relevant case management experience.

Qualifications

• New York State Driver's License is preferred.
• Experience with case management in housing or behavioral health is desirable.
• Ability to follow program plans and maintain documentation.

Responsibilities

• Provide case management for residents and develop daily living skills.
• Coordinate services: medical, psychiatric, entitlements, along with referrals.
• Visit clients in their homes or alternate locations multiple times per month.
• Maintain and document progress notes in AWARDS within 24 hours.
• Assist with safety and environmental assessments in the apartments.
