Job summary

Cleveland Clinic in West Palm Beach, FL is seeking an RN for the Rapid Response Program to provide professional nursing care across ambulatory settings, supporting caregivers in delivering exceptional patient experiences and coordinating timely transfers to Weston Hospital.

The role requires an ADN or BSN, current RN license, and BLS; ACLS is preferred for certain procedures. Part-time, day shift, outpatient experience preferred.

Qualifications

• Graduate from an accredited School of Nursing (ADN or BSN) required.
• One year of RN experience required; two years preferred.
• Outpatient experience preferred.
• Current RN licensure in the state where practicing.
• BLS certification required; ACLS preferred for certain procedures.

Responsibilities

• Assess health status by completing a nursing assessment.
• Perform telephone triage.
• Notify physicians/LIPs of abnormal findings.
• Establish a care plan in collaboration with the patient, family, and healthcare team.
• Collaborate with care coordinators to ensure smooth transitions of care.
• Assist with specialized diagnostic and therapeutic procedures.
• Administer medications and treatments as ordered and monitor response.
• Initiate CPR and other emergency measures.
• Provide health education to patients and families.
• Participate in quality initiatives, process improvements, and research activities to improve patient outcomes and safety.
