Qualifications

• Bachelor's degree in Nursing required; RN licensure in WA or multistate.
• Master’s degree in nursing preferred.
• CCMC/ACMA/AAACN certifications required or obtained within 3 years (waived for staff hired before 8/1/2023).

Responsibilities

• Ensure safe transitions of care for patients and families.
• Advocate for patients with cultural sensitivity.
• Provide patient-centered education on diseases and wellness.
• Support self-management and goal setting for chronic disease.
• Coordinate community resources via nurse visits or contacts.
• Collaborate with leadership to plan resource utilization across the continuum.
• Supervise RNs, LPNs, CMAs and guide problem-solving as needed.
