Qualifications

• RN license with Oklahoma or multi-state (eNLC) required.
• Bachelor’s in nursing or MSW; relevant licensure or certification preferred.
• 3+ years in care coordination; 3–5 years supervisory/leadership experience.
• Proficiency with EHR systems and healthcare data.
• Strong communication, leadership, and collaboration skills.

Responsibilities

• Provide direction and leadership to Care Management in ambulatory clinics.
• Oversee daily operations: staffing, assignments, problem solving.
• Collaborate with clinicians, interdisciplinary teams, and external partners.
• Monitor patient care processes and drive evidence-based improvements.
• Ensure compliance with healthcare regulations and quality standards.
• Lead performance improvement initiatives to enhance care quality and efficiency.
• Analyze data with the director to identify trends and opportunities.
• Build relationships with staff and stakeholders to support patient-centered care.
