Pacific Medical Centers (PacMed) is seeking a Supervisor Care Management RN for Beacon Hill, WA. This full-time, hybrid role leads the day-to-day operations of the care management team and collaborates with inpatient and ambulatory clinicians to ensure cost-effective, high-quality care.

Qualifications

Responsibilities

• Provides leadership and supervision of the day-to-day clinical and administrative operations of the care management team.
• Supervises Care Management staff including nursing, social work, patient care coordinators and other non-clinical staff to achieve organizational goals.
• Collaborates with inpatient and ambulatory clinicians and care teams, PacMed and PCN leaders to support cost-effective high-quality medical services.
