Job summary

Providence Medical Foundation in Anaheim is seeking a Pharmacy Coordinator to support Ambulatory and Population Health Pharmacy teams. You will review charts to extract data for reports and patient interactions, coordinate care by phone and through new technologies, and document workflows for care gaps.

Ideal candidates have strong organizational and analytical skills, good communication, and proficiency with Microsoft Office.

Qualifications

• Completed coursework in Medical Terminology or working knowledge of Medical Terminology.
• California Pharmacy Technician License upon hire.
• 2 years Pharmacy Technician experience.

Job description

Under direct supervision of management, this position is responsible for supporting the Ambulatory and Population Health Pharmacy teams. This pharmacy coordinator will have responsibilities that include reviewing charts to extract essential information for reports and for proper patient interactions, coordinating patient care by phone and new technologies, documenting workflows for pharmacy care gaps and patient coordination, assisting with our pharmacy student program, initiating and maintaining operational and quality reports, evaluating trends to report out to supervisors, assisting with prior authorizations, and support general pharmacy-related issue resolution under the guidance and direction of Pharmacists.

The ideal candidate demonstrates strong organizational and analytical skills, excellent friendly communication abilities, and proficiency in Microsoft Office applications and other computer-based technologies.

Required Qualifications:

• Coursework/Training:Completed coursework in Medical Terminology or has working knowledge of Medical Terminology.
• Upon hire: California Pharmacy Technician License.
