Job summary

HealthPartners is seeking a Pharmacist to join the MTM team in the ambulatory setting. The pharmacist will be responsible for safe and effective drug therapy and medication management to improve patient outcomes.

The role involves collaboration with a multidisciplinary team and direct patient care. Qualification requirements include a PharmD, Minnesota license, ambulatory care experience, and the ability to travel between clinic locations.

Qualifications

• Graduate from an accredited college of pharmacy on or after May 1996 or complete ACPE-accredited program.
• Two years of ambulatory care clinical pharmacy experience within the last five years or completion of a 1-year post-doctoral residency.

Responsibilities

• Provide MTM and medication management services as part of a multidisciplinary team.
• Improve population-based and patient-specific medication outcomes in ambulatory care.
• Collaborate with physicians, nurses, and other staff to coordinate care.
